NHDD - New York State Coalition

The New York State Liaison for 2010 is Dr. Patricia Bomba.  The NHDD NYS Coalition did an outstanding
job across New York State in 2009.

Goals for New York State's NHDD efforts include:

  • Increase conversations that lead to completion of health care proxies
  • Increase awareness of the MOLST Program in the community
  • Ensure that the Family Health Care Decisions Act is passed in 2010

In order to meet statewide goals we need volunteers to:

  • Serve as a regional representative (Dr. Bomba will oversee, assist and coordinate the Capital District area activities for NHDD) and/or
  • Lead/or be an active participant on a subcommittee (e.g. - ACP in High Schools, Colleges and Universities). 

The NHDD NYS Coalition will host monthly conference calls to share information regarding:

  • Health care reform with special attention to the provision in H.R. 3200 for thoughtful advance care planning discussions and MD reimbursement.
  • Updates on statewide implementation of the MOLST Program.
  • Strategies to ensure 2010 is the year Family Health Care Dcisions Act finally becomes law in New York State!
  • Plans for National Healthcare Decisions Day 2010

We are working on the Spanish translation of the revised Advance Care Planning Booklet.  We have been approached to work on both a Chinese and a French translation as well.  If you are interested in helping translate the ACP Booklet to other languages, please contact us.

 

We have co-branded the revised Advance Care Planning Booklet with the NYSBA.  If your organization is interested in collaborating to produce the ACP Booklet, please contact us.

Employee & Community Surveys
  • Since 2001 Excellus BlueCross BlueShield has done serial surveys about advance care planning for both its employees and the Upstate New York community.  The serial employee surveys have demonstrated increasing rates of awareness about advance care planning and increasing rates of health care proxy completion, from 30% completion in 2002, to 34% completion in 2006, to 43% completion in 2008.  These surveys have repeatedly highlighted the disparity between values and actions when it comes to end-of-life care planning.  The 2008 community survey found that while 90% of Upstate New Yorkers believed that it was important to have someone make medical decisions for them if they were unable to, only 42% designated a "health care proxy" to ensure their wishes are carried out. 
  • Independent Health did a survey of its employees for NHDD in 2009.  View the survey tool here.
    Employee Advance Care Planning Campaigns
  • In 2007, Excellus BlueCross BlueShield initiated an Advance Care Planning Campaign for its employees.  After the campaign there was an increase in completion of health care proxies from the year before.  Employers: please Contact Us if you would like more information about the Employer Advance Care Planning Campaign or would like a copy of the Employer Advance Care Planning Campaign Toolkit.
