Key Points

  • It is distributed as a BRIGHT PINK, cardstock, multi-page form, but it can be photocopied and faxed while remaining a medical order and legally binding.
  • It may replace a NYS non-hospital Do Not Resuscitate (DNR) form.
  • The NYS non hospital DNR form is still valid.
  • It includes information to be used in other health care settings such as the hospital (i.e. placement of feeding tubes, and so on).
  • Remember to ask is the MOLST form is present.
  • Add "D" to the SAMPLE acronym:
    • S - Signs and Symptoms
    • A - Allergies
    • M - Medication(s)
    • P - Pertinent past medical history
    • L - Last oral intake
    • E - Events leading up to contacting 911
    • D - Do you have advance directives and medical orders
            (i.e. non-hospital DNR or MOLST)?
