Medical Orders for Life-Sustaining Treatment - Professionals

MOLST Training Videos

The New York State Department of Health (NYSDOH) revised the MOLST for (DOH-5003) in June of 2010 to make it more user-friendly and to align the form with the procedures and decision making standars set forth in the Family Health Care Decisions Act (FHCDA).

In light of this, some sections of the MOLST Training are obsolete and under revision.  Steps 1 - 3 provide up-to-date MOLST and FHCDA information and education.

The MOLST DVD is a statewide initiative presented by the Medical Society of the State of New York and Excellus BlueCross BlueShield in collaboration with the New York State Deparmtne of Health and the Monroe and Onondaga Counties MOLST Community Implementation Team.
