Education for Physicians on End-of-life Care (EPEC)
Through the support of a grant from The Robert Wood Johnson Foundation, the American Medical Association developed the Education for Physicians on End-of-life Care (EPEC) curriculum.  The goal of the EPEC project is to close the educational gap and improve competency and confidence of healthcare professionals in caring for the dying.
Advance Care Planning
Community Conversations on Compassionate Care (CCCC)
Advance Care Planning Workshop designed as a facilitated group discussion and presentation led by trained lay and health professional staff.  The goal of these Community Conversations is to promote conversations that help ensure that wishes are honored.

Community Conversations on Compassionate Care Facilitator Training
CCCC Facilitator Training is an 8 hour training on Advance Care Planning. The training provides attendees with the tools needed to facilitate the CCCC workshop and trains attendees who deal with advance care planning on a daily basis with knowledge needed to facilitate 1-on-1 discussions.  The goal is to develop healthcare and community collaborations to promote advance care planning and the completion of advance care directives for all individuals.

Medical Orders for Life-Sustaining Treatment (MOLST)
MOLST is a program designed to improve the quality of care seriously ill patients with advanced chronic illness receive at the end of life. MOLST is based on antecedent goal-based, patient-centered conversation among patients, families and providers. Medical orders are recorded on a bright pink form that travels with the patient and is honored at all sites of care.
