Public Service Announcements Videos

The Medical Society of the State of New York (MSSNY) is pleased to present 30-second public service announcements (PSAs) to air across New York State demonstrating "conversations change lives" and the importance of advance care planning for all individuals 18 years and older. The PSAs are sponsored by a generous grant from MSSNY and Excellus BlueCross BlueShield.

Community Conversations on Compassionate Care (CCCC) Videos

Ten 2-minute clips, the 15-minute Five Easy Steps video, and the full 43-minute CCCC video illustrate stories from real patients and families and explain the benefits of the advance care planning process using Five Easy Steps. The CCCC Video Library also features the Community Partners in Advance Care Planning Education video, which illustrates the collaboration with Chi Eta Phi Sorority, Beta Chi Chi Chapter, and Rochester ministers.  These videos aim to motivate conversations among families and increase the completion rates of advance directives, specifically health care proxies.

Five Easy Steps Video

A 15-minute video outlines Five Easy Steps for completing the advance care planning process.

This video is appropriate for anyone (patients, families and professionals) who wants to learn more about advance care planning and complete traditional advance directives, such as a health care proxy.

Medical Orders for Life-Sustaining Treatment (MOLST) Videos

Learn about the MOLST Program through two key videos (one for patients, families and professionals, and one just for professionals) and three "refresher" videos.

• Writing Your Final Chapter: Know Your Choices… Share Your Wishes highlights five real stories that describe the value of the MOLST Program to everyone – patients, families and professionals.
• Honoring Patient Preferences: The Role of MOLST in New York State instructs medical professionals about the MOLST Program and details the process of completing a MOLST form.
