Since the inagugural National Healthcare Decisions Day (NHDD) held April 16, 2008, more than one million advance directive forms have been requested from the National Hospice and Palliative Care Organization's Care Connections program.  In April 2009, over 80 national organizations and no less than 700 state and community organizations increased awareness of advance care planning and educating their communities about the importance of making healthcare decisions by hosting events.  More than 100 organizations from across New York State participated in the NHDD NYS Coalition in 2009.

NHDD is a grassroots effort to promote advance care planning and healthcare decision making.  All Americans, regardless of age or current health, are encouraged to voice thier wishes and take steps to ensure that their choices are known and honored.

Patricia Bomba, M.D., F.A.C.P. has been active with NHDD since the inaugural event.  She was the New York State liaison for 2009 and is now in 2010.  In 2009, she developed two coalitions; the NHDD Rochester Region Coalition and the NHDD NYS Coalition in support of planning and hositing events in each attendee's community, encouraging all adults to complete a health care proxy.  In addition, a subgroup was formed for the Family Health Care Decisions Act (FHCDA).  New York State is one of the only states that prohibit family members from making health care decisions for incapacitated loved ones if they have not completed a Health Care Proxy.  The subgroup works toward having the FHCDA legislation passed.
